Direct Support Professional (DSP)/Day Program Facilitator Full Time South Orange, NJ, US 30+ days ago Requisition ID: 1002 Job Purpose The Day Program Facilitator provides direct support to clients who participate in the Day Program including developing and facilitating groups, as well as, individual support. The facilitator documents all information required to determine the client's progress towards the achievement of group and individual participant goals. Responsibilities Create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan Understand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed Transport clients as assigned (able to drive a 15-passenger van) Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk‑taking Make preparation and serve lunch for attendees Provide crisis intervention, conflict resolution, and mediation as needed Participate in all staff meetings, trainings and client meetings as assigned Must check email and voicemail daily Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards Complete other departmental documentation Correct all documentation as directed by supervisor Attend all electronic health record (Foothold) training as required Adhere strictly to HIPAA requirements and all agency confidentiality guidelines Establish positive, balanced relationships with clients, as demonstrated by: treatment of individuals with respect and dignity and in an age and culturally appropriate manner; a commitment towards the individual's rights and responsibilities; and establishment of clear boundaries that allow for productive interactions Building and Maintaining Positive Relationships Demonstrating Professional Ensuring Safety Both on Site and In the Community Supporting Independence and Growth Other responsibilities that may arise within the job scope Skills and Abilities The ability to communicate clearly both orally and in writing Flexible and able to work in a fast‑paced environment Self‑starter who can innovate new ideas and strategies for helping clients reach goals and outcomes Patience when working with individuals across a spectrum of intellectual and developmental disabilities Computer Skills Reporting N/A Educational Requirements High School Diploma or GED Associate degree or further degree in psychology, mental health, or a related field preferred Work Experience Requirement Minimum of two years working with individuals with developmental disabilities, or an equivalent combination of education and experience that provides the knowledge, skills and abilities to perform job duties and responsibilities proficiently. Position Type/Expected Hours of Work Must be available Monday through Friday 8am – 5pm. Valid New Jersey Driver’s license is required in good standing. Special Working Conditions/Physical/Requirements – Working Environment This job operates in a professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be available for coverage on some weekends and after 5pm when required. Must hold CPR/First Aid Valid Certification Successful clearance of NJ background check. Successful clearance of Drug Screening. Has fulfilled the necessary training requirements and is in good standing in order to bill Medicaid for Support and Community Care Waiver (CCW) Services and CDS.
