The Cardiac Rehab Nurse coordinates the care of Phase I Cardiac Rehab patients at the hospital. This position is Casual meaning the hours per week are not guaranteed. The hours will vary based on staffing needs but would be between 6:30am-6pm Monday, Wednesday and Friday. Tuesday and Thursday 7:30am-3pm. No Weekends or Holidays. This position will cover Delnor and CDH (Wheaton) Cardiac/Pulmonary Rehab locations.

Responsibilities:

• Provides patient initial assessment & orientation.
• Develops rehabilitation plan that addresses the patient's risk stratification, risk factors & exercise/functional status.
• Develops patient rehabilitation discharge goals that promote transitioning the patient back into community health & exercise resources.
• Communicates rehabilitation plan & discharge to ordering physician(s).
• Provides & documents patient care per department policy & external regulatory requirements.

Required:

• Bachelor of Science in Nursing
• BLS
• ACLS (within 6 months of hire)
• Current state of IL RN License in good standing
• Basic computer knowledge
• Ability to communicate effectively in English, both verbally and in writing
• Excellent customer service skills

Preferred:

• One year of experience in pertinent clinical specialty
