Calvert Internal Medicine Group is seeking a qualified Clinical Coordinator to join our Population Health team.

The Clinical Care Coordinator works closely with the RN Care Manager and Population Health team to coordinate patient care, identify patients who may benefit from care management, conduct patient outreach, address gaps in care, and connect patients with appropriate healthcare and community resources.

Primary Responsibilities

The Clinical Care Coordinator will:

Partner with the site RN Care Manager to identify patients appropriate for care management.

Conduct patient outreach and follow-up.

Coordinate patient care and assist with transitions of care.

Review and address gaps in care.

Support CIMG quality initiatives, MIPS measures, and Value-Based Care requirements.

Document patient care activities accurately in the EMR.

Communicate effectively with patients, providers, coworkers, and outside healthcare/community organizations.

Assist with triage and emergencies as needed.

Maintain patient confidentiality.

Participate in Clinical Care Coordinator, interdisciplinary, and Population Health meetings.

Maintain knowledge of community resources and healthcare organizations.

Perform other duties as assigned.

Required Qualifications

CMA or RMA certification.

Three or more years of experience in a primary care ambulatory setting.

Current CPR certification.

Strong communication, organizational, and customer service skills.

Ability to work effectively with the RN Care Manager and multidisciplinary team.

Ability to manage multiple priorities and maintain accurate documentation.
