Medical Records Clerk

This position is responsible for managing, organizing and maintaining patient health records, ensuring accuracy, confidentiality and accessibility for healthcare providers.

Communication: Effective written and verbal communication skills to interact with staff, patients, and external agencies Problem-Solving: Ability to investigate discrepancies and resolve issues creatively and efficiently PHYSICAL REQUIREMENTS:
ESSENTIAL DUTIES/RESPONSIBILITIES: Record Management:
- Organize and maintain patient records in both electronic and paper formats, ensuring they are up-to-date and complete.
- Accurately input patient information, including demographics, medical histories, treatment details, and test results into electronic health record (HER) systems.
- Retrieve medical records for healthcare providers, patients, or authorized personnel as needed
- Ensure all records comply with HIPPA and other federal regulations, maintaining confidentiality and security of patient information.
- Maintain confidentiality of all patient information at all times
- Coordinate with medical staff, insurance companies, and patients to clarify information, obtain additional documentation, or respond to inquiries.
- Monitor and execute requests in work queue that result in processing requests for information
- Audit records for accuracy, completeness, and compliance with institutional policies
- Support audits by providing requested documentation promptly and accurately
- Perform administrative tasks such as answering phones, processing records requests, scanning documents, and other duties as assigned/required by the role.

- Moderate physical exertion including frequent walking, standing, bending, reaching and carrying moderately heavy loads (up to 25 lbs.)
