About the Role:

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health and Hospice team in Fresno, CA. As a Registered Nurse in Home Health and Hospice, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their safety, comfort, and well-being. You will work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for each patient, and provide education and support to their families. Your ultimate goal will be to help patients achieve their highest level of independence and quality of life.

Minimum Qualifications:

• Active and unrestricted Registered Nurse license in the state of California
• Minimum of 1 year of recent experience in Home Health or related field
• Current CPR certification
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Responsibilities:

• Answer all after hours and weekend phone calls.
• Triage patient needs - If issue cannot be resolved via phone/facetime then a skilled visit will need to be performed for Home Health calls. Most hospice issues/incidents will need a skilled visit.
• Perform after hours/weekend hospice admissions as needed.
• Coverage area will be the Fresno and Visalia Area Territories.
• Updating on-call chat and patient charts as needed.

Skills:

As a Registered Nurse in Home Health and Hospice, you will utilize your clinical expertise, critical thinking, and problem-solving skills on a daily basis. You will need to be proficient in administering medications, treatments, and procedures, as well as in assessing and documenting patients' health status and progress. Excellent communication and interpersonal skills are essential, as you will be working closely with patients, families, and other healthcare professionals. You will also need to be able to work independently and as part of a team, and be comfortable with using technology and electronic medical records to manage patient care.
