RN Wound Ostomy Nurse

The Registered Nurse with certification in Wound, Ostomy, and Continence Nursing (WOCN) serves as a consultant to staff and patients regarding skin prevention, wound care, ostomy management, and continence needs. This role involves collaborating with the healthcare team to provide specialized nursing care, promote patient comfort, and support optimal healing outcomes. The nurse must demonstrate strong communication skills, professional demeanor, and at least three years of medical-surgical nursing experience. Responsibilities:

• Provide expert consultation and assessment for skin prevention, wound care, ostomy, and continence management.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients, families, and staff on wound and ostomy care techniques and best practices.
• Monitor patient progress and adjust care plans as needed to promote healing and prevent complications.
• Serve as a resource and liaison between patients and healthcare staff to ensure coordinated care delivery.
• Maintain accurate and timely documentation of assessments, interventions, and patient outcomes.
• Demonstrate professional communication and interpersonal skills with patients, families, physicians, and co-workers.

• Active Registered Nurse license in the State of Pennsylvania.
• Board certification in Wound, Ostomy, and Continence Nursing (WOCN).
• Minimum of three years full-time professional nursing experience with medical-surgical patients.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal, non-verbal, and written communication skills.
• Ability to maintain professional attitude, sound judgment, and emotional tolerance in patient care.

• Experience functioning as a liaison between patients and healthcare teams.
• Advanced knowledge in skin prevention and continence care.
• Demonstrated ability to support harmonious communication within multidisciplinary teams.

• Must maintain professional composure and sensitivity when interacting with patients and families.
• Role requires tact, sound judgment, and emotional resilience in a clinical setting.

• Day shift schedule.
• Work hours and shift assignments to be determined by the facility's staffing needs.
