FCP Case Manager

Humana is seeking an FCP Case Manager in Milwaukee, WI to coordinate comprehensive care for members with complex medical and social needs. In this role, you will manage a caseload, conduct holistic assessments, and develop individualized care plans that integrate medical, behavioral, and community resources. You will collaborate closely with providers, community partners, and families to close gaps in care, address social determinants of health, and support members in navigating benefits and services.
