The LPN will function as part of an interdisciplinary team under the supervision of the Patient Care Manager and will perform patient visits as assigned by the RN Case Manager. The LPN will play a vital role in the coordination of care of the patient to ensure their loved ones, facility partners, and hospice team are aware of any changes or needs.
