Licensed Practical Nurse

Provide direct skilled nursing care to residents Supervise day-to-day nursing activities performed by nursing assistants Administer medications and treatments as ordered Monitor residents’ health and communicate changes to physicians and the healthcare team Collaborate with physicians, therapists, CNAs, fellow nurses, residents, families, and support departments Admit, transfer, and discharge residents Complete nursing documentation, charting, reports, forms, and care-plan updates Receive and transcribe physician orders Perform nursing procedures including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, and range-of-motion exercises Make rounds, monitor seriously ill residents, and ensure prescribed treatments are administered Coordinate diagnostic, therapeutic, social, activity, restorative, and rehabilitative services Provide leadership, staffing support, work assignments, performance feedback, and supervision to nursing personnel Participate in quality assurance, facility inspections, staff development, in-service training, infection control, safety, and disaster-preparedness programs Maintain adequate medication, equipment, and supply levels Review resident care plans and ensure care is provided according to residents’ needs and wishes Maintain a clean, safe, sanitary, and caring environment Report accidents, incidents, exposures, suspected fraud, unauthorized access, and changes in resident condition as required Requirements Nursing degree from an accredited college or university, or graduation from an approved LPN/LVN program Current, unencumbered, active South Carolina LPN/LVN license in good standing Current CPR certification Must read, write, speak, and understand English Knowledge of nursing and medical practices, procedures, laws, regulations, and nursing-facility guidelines Knowledge and experience with PCC preferred Must maintain required continuing education and licensing Must remain in good standing with the State Board of Nursing Ability to provide care appropriate to residents’ age-related needs Leadership and supervisory ability Ability to make independent decisions and work effectively with residents, families, personnel, visitors, agencies, and the public Ability to plan, organize, develop, implement, and interpret nursing programs, policies, and procedures Ability to move intermittently throughout the workday and cope with mental and emotional stress Ability to push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry it at least 50 feet Ability to assist with resident evacuation during emergencies Must meet facility general health requirements, including medical and physical examination Must not pose a direct threat to workplace health or safety Willingness to work beyond normal hours, temporarily in other positions, and be subject to emergency call-back Core Competencies Demonstrates expertise in providing skilled nursing care, administering medications, and performing nursing procedures while ensuring compliance with healthcare regulations and facility guidelines. Exhibits strong leadership and supervisory skills to effectively manage nursing personnel and coordinate resident care.
