**This position will be primarily based out of Jefferson with travel required to Guthrie Center. It is a mixture of Day Habilitation Services and Supported Employment Services**

This role operates in a Day Habilitation (DAY HAB) Setting , defined as services "that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment." (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. It will also operate in a Supported Employment (SE) Setting , defined as services "that enables the member to obtain and maintain individual employment" (Iowa Health & Human Services). Rather than assuming individuals with development or intellectual disabilities are not qualified to work, Supported Employment services provide these individuals with the opportunity to obtain and maintain meaningful employment that supports their life goals.

As an integral part of the overall supported employment team, you will collaborate with other Community Integration Professionals, Direct Support Professionals, Programmers, Supervisors, individuals-served, and their families to ensure the best possible care for those you serve. Your innovative and impactful solutions and strategies will make the difference for those you serve.

• Person-Centered Community Engagement: Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
• Goal-Based Skill Development: Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction rather than doing tasks for them.
• Behavioral Support Implementation: Follow individualized behavioral care plans by applying approved strategies, encouraging participation, motivating progress, and supporting positive behavior in alignment with each person's goals.
• Documentation & Progress Tracking: Maintain accurate, consistent, and timely documentation of individual progress, activities, and outcomes to ensure continuity of care and accountability.
• Safety & Advocacy: Provide a safe, supportive environment while respecting individual choice and autonomy, advocating for independence, dignity, and person-directed decision-making in all aspects of service delivery.
• Vocational & Community Employment Support: Support individuals in job development and retention, including applications, interviews, coaching, employer communication, transportation to job sites, and collaboration with schools, IEP teams, and community partners.
• Program Participation & Professional Responsibilities: Participate in cross-training, vocational program decisions, community integration activities, equipment safety, medication administration (as applicable), compliance with all regulations (OSHA, DHS, CARF, HCBS), and maintain flexibility for scheduling, weekends, and additional duties.

You will need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage - don't worry, we can answer any questions you might have about this.
• Education Requirement: While no higher education is necessary, we do look for candidates who have obtained their diploma or the equivalent representing their high school education.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

• May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
• May require prolonged sitting and working at a computer for up to eight hours.
• May require frequent bending, stooping, and stretching.
• May require prolonged standing for up to eight hours.
• Must wear hearing and eye protection in required areas.
• Must have ability to see objects/persons at a distance.
• Must have ability to see close work such as typed or handwritten material.
• Must have ability to hear conversations in a quiet and a noisy environment.
• Must have ability to determine where a sound is coming from.
• Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
• Must have ability to communicate through speech.
• Must have ability to drive and transport for service provision if needed.
• Must remain awake during working hours, if applicable.
