Edgewood Center for Rehab is seeking a motivated and compassionate Per Diem Physical Therapist (PT) to join our rehab team.

Our beautiful 5-star skilled nursing facility in historic Portsmouth provides exceptional short-term rehabilitation and long-term care services in a supportive, team-focused environment. We are a forward-thinking organization that values both our employees and the residents we serve.

• Graduate of an accredited Physical Therapy program

• Current NH Physical Therapist license in good standing

• Long-term care and/or geriatric experience preferred

The Physical Therapist evaluates residents, develops individualized treatment plans, and provides skilled therapy services focused on restoring function, improving mobility, reducing pain, and maximizing independence.

• Evaluate residents and establish individualized treatment plans

• Provide skilled physical therapy interventions and therapeutic exercises

• Complete accurate and timely documentation

• Collaborate with interdisciplinary team members to support resident outcomes

• Monitor resident progress and modify treatment plans as appropriate

• Educate residents, families, and staff on mobility, safety, and exercise programs

• Assist with discharge planning and equipment recommendations

• Maintain a safe, clean, and organized therapy environment

• Support a positive and collaborative rehab culture

• Flexible per diem scheduling
