Overview

We are seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our growing healthcare team. In this role, you will serve as the clinical lead for your patients by performing comprehensive assessments, coordinating individualized care plans, and ensuring high-quality outcomes in a home health setting .

The ideal candidate has experience with Home Health services, OASIS assessments, and Start of Care (SOC) visits .

Key Responsibilities

Patient Assessments

• Perform comprehensive patient evaluations, including Start of Care (SOC), recertifications, and follow-up assessments
• Complete accurate OASIS documentation in compliance with home health guidelines

Care Coordination

• Develop, implement, and update individualized patient care plans
• Coordinate care with physicians, caregivers, families, and interdisciplinary team members

Documentation & Compliance

• Maintain accurate and timely clinical documentation
• Ensure compliance with company policies, quality standards, and regulatory requirements

Patient Advocacy

• Educate patients and families regarding care plans, treatment goals, and available resources
• Serve as the primary clinical point of contact throughout the patient’s care

Qualifications

Required:

• Active and unrestricted Registered Nurse (RN) license
• Strong clinical assessment and communication skills
• Ability to manage patients independently in a home-based setting

Preferred Experience:

• Previous Home Health RN Case Manager experience
• Experience completing OASIS assessments
• Experience performing Start of Care (SOC) visits, recertifications, and admissions
• Experience with Medicare-certified home health services

Preferred Certifications (Not Required):

• COS-C (Certified OASIS Specialist – Clinical)
• HCS-O (Home Care Clinical Specialist – OASIS)
• HCS-D (Home Care Coding Specialist – Diagnosis)
