Our programs specialize in preventing family homelessness when possible, providing shelter and transitional housing to families in times of need, supporting the wellbeing and academic achievement of children who experience homelessness, and quickly returning families to safe and stable housing throughout the Greater Bay Area. What You’ll Do

We’re excited to be adding a Case Manager to our team! This is an engaging and rewarding opportunity that plays an integral role within the organization. We are looking for someone who is passionate about working with our participants and contributing to the fight against homelessness. The Case Manager will report directly to the Family Services Manager and be part of the Shelter Program team. This position provides case management to families in the shelter program using various service models. This involves documenting services in real-time, maintaining electronic case records, and collecting monthly compliance reports.

What Your Day Looks Like

  • Offer personalized case management to participants, guiding them through intake, assessments, and tailored service plans that meet their unique needs
  • Connect individuals with vital community resources, advocating for housing, employment, and support in areas like mental and physical health, domestic violence recovery, and family reunification
  • Design and facilitate dynamic workshops that empower participants with essential life skills and strategies for crisis intervention
  • Welcome families during intake and orientation, collaborating to create actionable plans that address housing, recovery, and children’s needs
  • Build strong networks with community services to ensure participants receive the support they deserve
  • Work closely with case managers, supervisors, and community partners to provide seamless and effective support for families
  • Uphold program policies while keeping clear and accurate records of participant progress and experiences
  • Prepare and submit insightful reports that highlight the impact of services and track our program's success
  • Actively participate in team meetings, training sessions, and community events to strengthen our mission
  • Ensure compliance with all regulatory requirements and organizational policies, and attend hearings as needed
  • Encourage participant involvement in decision-making and program activities, fostering a sense of ownership
  • Take on additional responsibilities as requested to support our mission

What Makes You Successful in This Role

  • Ability to work and collaborate with a diverse group of people on projects and making decisions for the benefit of our participants
  • Familiarity with case management databases or customer service platforms
  • Highly organized, self-motivated, and capable of working independently to meet goals
  • Demonstrated ability to exercise appropriate judgement and uphold policies and procedures
  • Skilled in documentation, reporting, and maintaining confidentiality

What You Bring

  • 1+ years of experience in social work, psychology, human services setting or related field
  • A background in working with nonprofit organizations, social services organizations, or community-based programs
  • Familiarity with Bay Area resources for low-income families
  • Bilingual abilities, especially in Spanish
  • Highly proficient in Microsoft Office Suite, Salesforce, and other software
  • Ability to travel locally and long distance to meet with vendors and participants

Salary Range: $28.00/ hour

Bilingual Premium (Spanish): $1.50 extra per hour

Position Type: Full-time, Non-Exempt

Union Representation: Yes

Schedule: 40 hours with evenings and/or one weekend day required (10:00am to 6:30pm or 10:30am to 7:00pm)
