We are seeking a compassionate and experienced Per Diem Registered Nurse to join our dedicated team at A Place At Home Somerville. As a Per Diem RN, you will provide high-quality, patient-centered care to our clients in the comfort of their own homes. About this Role:

• Conduct initial and ongoing assessments to identify client needs and develop effective care plans.

• Collaborate with clients, families, and caregivers to ensure seamless transitions and optimal outcomes.

• Conduct caregiver supervisory visits in compliance with state regulations and agency policies.

• Assess caregiver competency and provide coaching, education, and support as needed.

• Utilize strong communication and interpersonal skills to manage client expectations and maintain positive relationships

Requirements:

• Active RN license in the state of NJ

• Bachelor’s degree in Nursing (preferred)

• Minimum 2 years of clinical experience, ideally in home care

• Reliable transportation and auto insurance

• Clean background and criminal record

• CPR/BLS certification
