Description: Monday - Friday 8:00 AM - 4:30 PM; weekends as needed. Mileage can be reimbursed. Provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. Assists in evaluating the level of function and helps develop the plan of care while revising it as necessary. Prepares clinical and progress notes and consults with the family and other personnel. Participates in in-service programs. Promotes effective working relationships as part of the team to meet goals and objectives. Utilizes resources, supplies, and time efficiently. Promotes positive, supportive, and respectful communication to all external sources and team members. Strives for continuous improvement in aspects of the Physical Therapist function and Care Delivery process. Develops and maintains a customer-oriented environment responsive to the needs of all customers. Direct Patient Care Duties: Develops and revises treatment plans and discharge plans in conjunction with patients, families, and care delivery teams. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiates appropriate rehabilitative programs and provides supervision to ensure patient health, safety, and compliance. Consults with and educates patients and families regarding disease processes, self-care techniques, and preventive strategies. Implements home therapy programs including gait training, transfers, activities of daily living (ADLs), and equipment use. Periodically evaluates patient progress and makes necessary adjustments to physical therapy programs. Instructs para-professional staff in body mechanics, equipment use, bed mobility, and home exercise programs. Directs, coordinates, evaluates, and supervises the quality of patient care services provided. Interacts with patients and others in a manner conducive to positive relationships. Participates in scheduled patient conferences for discussing patient care and treatment plans. Performs other duties and responsibilities to support the efficient operation of the team and discipline. Ensures completion of all appropriate clinical records needed for compliance with regulations. Maintains current knowledge of available community-based services. Makes constructive recommendations to improve quality of patient care and service. Ensures that patient needs are met through the care delivery process. Participates in educational opportunities and self-development as needed. Demonstrates commitment to professional growth and competency. Participates in professional organizations and shares acquired knowledge with team members. Responsible for meeting established Key Performance Indicators (KPIs).
