Primary Care Physician

Their integrated care model includes primary care, pharmacy, dental, behavioral health, prevention programs, and community health initiatives.Our client is seeking a Family Medicine or Internal Medicine Physician (MD/DO) to join their outpatient care team. This provider will deliver comprehensive primary care services including patient evaluations, diagnosis, treatment planning, and ongoing disease management.
