NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. We believe that people can change, and we work to empower each individual in their own care.

We are currently exploring a Bi-lingual Case Manager candidates in our Family Wraparound program. The Family Wraparound Program is a short term comprehensive General Preventive Services program that focuses on the following areas of the Child and Family Services Continuum: community supports; early interventions; home-based responses; and post-placement re-integration for children. The Program adds a strong clinical component for family and individual treatment. All Wraparound program services take place within the community and include case management, in-home therapy, and respite services.

Responsibilities:

• Provide support and guidance to the program families and youth around service plan goals Primarily responsible for providing linkages to community services to the family to ensure that they receive benefits, entitlements and needed services
• Provide case management counseling to youth in the program as it relates to service goals, through meeting with the family at minimum twice monthly in a community location Assists with group activities, with childcare during support groups, and with respite, tutoring and mentoring when needed
• Provide transportation support for families to attend medical appointments and/or court, as needed Timely completion of documentation in accordance with NAFI and OCFS standards and policies

Desired Qualifications:

• Must hold a valid driver's license, have own vehicle and willingness to travel throughout Orange County and to travel periodically to offices in Westchester County, and other locations as required to fulfill job responsibilities.
• Bilingual (English / Spanish)
• Bachelor's degree 1-3 years of experience and/or training; or Equivalent combination of education and experience

Hourly Rate: $23.00 - $24.00 per hour
