As an In-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team. Master's Degree in Nursing from an accredited program. Strong clinical background in primary care and preventive medicine. Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings. Ability to engage and communicate effectively with elderly and chronically ill patients and their families. Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target. Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus. Key Responsibilities: Conducting new admission evaluations and monthly stability visits for long-term residents Performing comprehensive physical examinations and evaluating changes in patient conditions Prescribing medications and treatments as needed Developing patient care policies and facilitating referrals to specialists Coordinating advanced care planning and discharge visits Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups Supporting the physician-patient relationship and addressing gaps in care Salary & Compensation: Annual Pay : $105,000 - $130,000 based on experience Monthly Bonus Opportunities : Monthly performance-based bonuses Benefits: Health, Dental, and Vision Insurance 401(k) plan 22 days of Paid Time off annually CME Stipend and CME PTO Up-to-date Malpractice & Tail Insurance coverage provided Apply today and make a difference in the lives of our patients and their families!
