Therapist - Social Worker

Job Description

Job Summary:
Provide clinical social work services to patients in an acute hospital setting, ensuring compliance with MediCal and Medicare regulations. Collaborate with healthcare teams to support patient care plans and discharge planning.

Responsibilities:

• Assess patients' psychological and social needs to develop appropriate care plans.
• Support patients and families through emotional and social challenges related to hospitalization and treatment.
• Document assessments and interventions accurately within clinical records.
• Coordinate with medical staff to facilitate smooth patient transitions and discharge processes.
• Maintain up-to-date knowledge of MediCal and Medicare requirements impacting care delivery.

Qualifications:

• Possess a valid California LCSW license.
• Have at least one year of acute care hospital experience; two years preferred.
• Be familiar with MediCal and Medicare policies and requirements.
• Demonstrate proficiency in Cerner electronic health record system (strongly preferred).
• Strong communication and interpersonal skills to collaborate with multidisciplinary teams.
