Physical Therapist Per Diem

Visiting Nurse & Hospice of Litchfield County, a non‑profit home care & hospice agency serving the community for over 100 years has openings for per diem Physical Therapists in Litchfield County and Farmington Valley service area. Strong teamwork environment, flexible scheduling, ability to work independently. QUALIFICATIONS Must be a registered physical therapist currently licensed to practice physical therapy by the State of Connecticut, and qualified and approved to treat Medicare patients. Must have completed at least a baccalaureate degree program in an accredited college or university and have had a specified period of clinical education. Must have at least two years of clinical experience in physical therapy, preferably with at least one year's experience in a home health care agency or related community health programs. Must be competent in physical assessment skills. Must have dependable and properly insured transportation. Must have a valid and current driver’s license. Must be able to read, write and communicate in English. Must be accessible by telephone and be available to make home visits. Must have the interpersonal skills necessary for working with patients and caregivers, the public and with medical managers, as well as in a group/team environment.
