Responsibilities
- Conduct psychosocial assessments for patients and families to identify needs and discharge planning requirements.
- Develop and implement patient- and family-centered care plans; coordinate with the care team and community resources.
- Provide counseling and support to patients and families facing illness, hospitalization, and transitions of care.
- Facilitate referrals to social services, financial assistance, housing, transportation, and community programs as needed.
- Document assessments, plans, and progress in the patient record; ensure privacy and regulatory compliance.
- Participate in multidisciplinary rounds and discharge planning meetings to support safe transitions.

Requirements
- Active Georgia LCSW license
- Minimum 2 years of experience in medical social work (acute care preferred)
- Acute care experience required
- Ability to work in an acute care environment
- Strong communication and documentation skills, team collaboration
