Clinical Nutrition Manager (RD)

Salary: Up to $95,000 per year
Luxor at Mills Pond Rehabilitation & Nursing Center is seeking a knowledgeable and collaborative Clinical Nutrition Manager to support residents’ nutrition and hydration needs. In this role, you’ll work with residents and the interdisciplinary care team to provide nutrition services and support individualized care planning.
ResponsibilitiesAssess residents’ nutrition and hydration needs and document findings.
Develop, implement, and update nutrition care plans in collaboration with residents and the interdisciplinary team.
Monitor residents’ weights, food and fluid intake, and other nutrition-related indicators.
Make recommendations for therapeutic diets, supplements, and food texture modifications as appropriate.
Communicate nutrition-related concerns and recommendations to residents, families, and care team members.
Support the center’s dining and food service operations, including menu review and meal quality monitoring.
Maintain accurate records and support compliance with applicable care and documentation requirements.
Participate in quality improvement activities, audits, and survey preparation.
Current New York State certification as a dietitian-nutritionist required.
Registered Dietitian (RD/RDN) credential preferred.
Experience in long-term care, rehabilitation, or a similar healthcare setting preferred.
Knowledge of clinical nutrition, therapeutic diets, and nutrition documentation.
Strong communication, organization, and teamwork skills.
Commitment to resident-centered care and confidentiality.
Salary of up to $95,000 per year , depending on qualifications and experience.
