Nurse Practitioner - Arlington Heights, IL

As a leading provider of remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM), we're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team.

Job Summary:

We're looking for a part-time Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine to deliver high-quality preventive care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As an integral part of our clinical team, you'll be responsible for building strong relationships with facilities, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities:

• Provide high-quality preventive care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Build strong relationships with facilities and maintain a patient-centered approach

• Be a collaborative team player, working closely with our clinical directors, regional managers, and supervisors

• Document patient care efficiently and accurately

• Make your own schedule and enjoy a flexible work arrangement

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

Salary: $17,000 - $19,000 ANNUALLY
