Hospice RN

Kaiser Permanente seeks a compassionate Hospice RN to provide high-quality, patient-centered end-of-life care. In this full-time role, you’ll manage a caseload of hospice patients, performing comprehensive assessments, developing individualized care plans, and coordinating services with physicians, social workers, and chaplains. You’ll administer medications, monitor symptoms, and educate families on care techniques and resources. Leveraging modern EMR and communication tools, you’ll document care accurately and collaborate with a supportive, innovative team focused on continuous improvement and meaningful patient outcomes.
