MDS Coordinator

Grand Islander RI and V2U, part of Genesis HealthCare, seeks an MDS Coordinator RN to lead resident assessment and care planning in our skilled nursing and rehab facility. You will complete and audit MDS assessments, coordinate interdisciplinary care plans, ensure accurate documentation, and support regulatory compliance and reimbursement. The role collaborates closely with nursing, therapy, and social services to promote resident-centered care, quality outcomes, and safety.
