Key Responsibilities

• Provide physical therapy services to patients in a home health and hospice setting
• Conduct patient evaluations and develop individualized treatment plans
• Travel to patient homes to deliver care, with daily mileage reimbursement provided
• Collaborate with an interdisciplinary care team to support patient goals and outcomes
• Document patient progress and maintain accurate clinical records

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington or ability to obtain prior to start
• Previous home health experience required
• No call requirement for this assignment
• Ability to begin as soon as possible following credentialing

Preferred Experience / Skills

• Experience with home-based physical therapy programs
• Familiarity with hospice care principles and patient populations
• Strong ability to work independently in a community/home setting
