TASC Care Manager

Position Title TASC Care Manager Location Pasquotank County (Elizabeth City, NC) and virtual services in Camden County. Hours 40 hours/week, full time, Monday–Friday, non‑exempt. Job Summary Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice‑involved individuals by providing case management and service coordination within the TASC (Treatment Accountability For Safer Communities) program. Responsibilities Administer psychosocial screenings and assessments. Coordinate linkages to behavioral health treatment, medical, educational, and vocational services as needed. Provide ongoing professional care management services for the justice‑involved population. Qualifications Education: Bachelor’s Degree or higher in a human services or related field. Certification: Preference given to individuals with North Carolina Peer Support Specialist certification and at least two years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB). Experience: Knowledge of recovery, harm reduction, and wellness‑based concepts; may have lived experience with recovery and/or the justice system. Required Skills Strong communication, organizational, and time‑management skills. Ability to work independently and manage a caseload effectively. Proficiency with the Microsoft Office Suite and similar platforms. Proficiency in using electronic health record systems and remote collaboration tools (Teams, Zoom, etc.). Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
