Therapist Physical Therapist

Job Summary:
Responsibilities:
Assess patients physical conditions and develop individualized treatment plans.
Implement therapy interventions to facilitate patient recovery and enhance mobility.
Document patient progress accurately and timely using HomeCare HomeBase EMR.
Educate patients and caregivers on proper exercises and activity modifications.
Coordinate with healthcare team members to ensure comprehensive patient care.
Travel up to 75 miles as required to deliver home health services in designated areas.
Qualifications:
Hold an active license in good standing to practice as a Physical Therapist.
Minimum of 1 year of recent adult home health experience.
Proficient in OASIS and experienced with HomeCare HomeBase EMR.
Reliable transportation, valid drivers license, and auto insurance.
CPR and BLS certification required.
Ability to comfortably handle a caseload of 5-6 patients per day.
