Aging-In-Place Community Social Worker
Presbyterian Senior Services
Presbyterian Senior Services (PSS) is seeking a NY Connects Social Worker to coordinate supportive services for older adults in Brooklyn. You will provide information on long-term care options, conduct eligibility screenings, and assist with enrollment in Medicaid, SNAP, and HEAP. Home visits and client advocacy are key components of this role.
