Schedule: Monday-Friday, 8:00 am-5:00 pm
Travel: Yes - between 4 local facilities during shift
Dress Code: Medical attire (no required scrub color) Job Description We are seeking a field-based Registered Nurse to support care coordination, clinical collaboration, and patient management across multiple long-term care and rehabilitation facilities within Pinellas County. This role is ideal for an RN who excels in interdisciplinary teamwork, enjoys working directly with facilities and providers, and is comfortable traveling locally during the workday. Primary Responsibilities

Primary Responsibilities
- Ensure accurate, complete, and timely documentation in patient charts
- Communicate and collaborate with the interdisciplinary care team across assigned facilities
- Assist providers with care coordination activities , including therapy and benefit determination
- Support benefit review and coordination related to Medicare Part A, Part B, and Part D
- Identify and respond to changes in patient condition , collaborating with the advanced practice clinician (APC)
- Participate in facility-based meetings , family conferences, and care team case reviews
- Support training, tracking, and compliance for quality measures
- Conduct in-services for facility staff as needed
- Assist providers in documentation accuracy, coding support, medication reconciliation, assessments, and nursing rounds
- Write verbal orders from a provider in the chart (in compliance with state RN practice laws and facility policies)
- Manage patients experiencing acute medical situations or exacerbations of chronic conditions
- Support patients during short-term sub-acute rehabilitation episodes
- Active and unrestricted RN license in the state of residence (or ability to obtain by start date)
- Associate degree or higher in Nursing
- Basic Life Support (BLS) certification
- Ability to lift a 30-pound clinical bag and perform hands-on physical assessments
- Reliable transportation to travel between facilities
- Full COVID-19 vaccination (required per federal, state, and facility requirements)
- Ability to pass a drug test
- 1+ year of clinical experience , ideally in a long-term care or rehabilitation setting
- Experience working with patients with complex medical, behavioral, or social needs
- Knowledge of geriatrics and chronic disease management
- Strong computer skills and ability to document in both written and electronic systems
- Effective communication, teamwork, and time-management skills
- Ability to travel between facilities throughout the day
- Familiarity with interdisciplinary care in multiple clinical settings
