Position Title: Physical Therapist (Inpatient and Outpatient)FLSA Status: ExemptDepartment: TherapyReports To: Manager of Therapy, Director of TherapyPOSITION SUMMARY:As a Physical Therapist (PT), you will help to ensure that REHAB patients and their families receive caring and dedicated service at REHAB. You will provide support for the Therapy Department and assist with a variety of services with a commitment to delivering high-quality, comprehensive and innovative inpatient and outpatient rehabilitation services. These services include interpreting physician’s orders, performing evaluations and assessments; designing and implementing plans of care; maintaining treatment and progress reports; drafting and/or presenting oral or written reports of patient’s status; collaborating with team members toward reaching patient’s goals.Wage Range: $83,208.32/year to $124,812.48/year.The actual wage is dependent on the applicant's relevant experience and qualifications for this position. The wage range for this position may be subject to change in the future depending on a variety of factors such as market conditions, business needs, legal developments, and other appropriate factors.REQUIRED QUALIFICATIONS:Certification/Licensure:Current State of Hawaii PT licensure.Current BLS certificationCompletion of competencies and other job-related and REHAB requirements.Education:Graduate from an APTA accredited school of Physical TherapySkills/Experience:Ability to safely and effectively manage patients’ mobility.Ability to clearly communicate in English, verbally and in writing.Ability to see and hear to safely provide care to patients.PREFERRED QUALIFICATIONS:Skills/Experience:For Inpatient: One (1) year physical rehabilitation experience.For Outpatient: Minimum two (2) years physical rehabilitation experience.Professional Affiliation:American Physical Therapy AssociationHawaii Physical Therapy AssociationCORE VALUESOur team models REHAB’s core values H.E.A.R.T. in daily actions. Teamwork – Work together for success.ESSENTIAL JOB FUNCTIONSEvaluates patient for therapy services.Develops, implements and modifies treatment plan.Delegates treatment activities to Physical Therapy Assistants (PTA), Rehabilitation Technicians and other support staff as appropriate. Provides instruction and monitors staff’s performance of activities.Documents timely and appropriately, in compliance with regulatory, third party payor and REHAB standards, and monitors PTA documentation on a timely basis.Actively involves patient/caregiver in assuring that mutual discharge goals are met.Recommends home modification and durable medical equipment (DME) needs, as appropriate.Completes prescription and assures receipt of appropriate DME/adaptive aids.Orients and trains patients/caregivers for self care skills.Follows and enforces Infection Control policies and procedures.Uses sound judgment and ensures patient safety.Follows and adheres to all organizational and departmental policies and guidelines, code of conduct, and REHAB employee handbook.Performs other duties as assigned.PROFESSIONALISM AND COMPLIANCE COMPETENCIESMaintain a high level of proficiency in the following REHAB competencies.Job KnowledgeAccountabilityCommunication and interpersonal/relationship building skillsAdaptability and Problem Solving, Decision MakingOrganization and Project ManagementEQUIPMENT, TOOLS, WORK AIDS USEDOffice equipment, including telephone, computer and multifunction devices (MFDs).
