Role Description This is a full-time, on-site role based in the Finger Lakes region, NY, supporting the delivery of patient-centered care within a collaborative clinical team. The role involves working directly with patients, accurately gathering and documenting health information in medical records, and helping coordinate appointments, referrals, and follow-up care. Team members support providers and other clinical staff in ensuring smooth patient flow, clear communication, and adherence to organizational policies and quality standards. The position also contributes to health education efforts, promotes wellness resources, and participates in continuous improvement activities to enhance patient experience and outcomes. Qualifications Spanish proficiency is required. A clean NYS driver's license is required. Clinical and patient care abilities, such as providing direct patient support, assisting with examinations or procedures (as applicable to the specific role), and following established care protocols. Administrative and organizational skills, including accurate documentation in electronic health records, managing schedules and referrals, and maintaining well-organized patient files. Communication and interpersonal skills, including clear verbal and written communication, active listening, and the ability to build respectful, trusting relationships with patients, families, and colleagues. Cultural sensitivity and community-focused skills, including working effectively with diverse populations, supporting equitable access to care, and promoting community wellness. Teamwork and collaboration abilities, including working in a multidisciplinary setting, responding flexibly to changing priorities, and supporting shared goals for quality and patient satisfaction. Relevant education or training for the role (for example, clinical certification, an associate or bachelor’s degree in a health-related field, or equivalent experience), and a commitment to ongoing professional development.
