Contract | Full-Time | Days | Sunday through Saturday work week

Key Responsibilities

• Provide physical therapy services across both skilled nursing facility (SNF) and home health settings on an approximately 50/50 basis
• Supervise and support two Physical Therapist Assistants (PTAs)
• Utilize SNF-based Net Health software and home health HCHB software for documentation and patient management
• Meet productivity expectations of 80% in the SNF setting and a points-based system of approximately 80 minutes per patient in the home health setting
• Assist with coverage in additional service areas as needed based on PTO and census demands

Required Qualifications

• Active Physical Therapist (PT) license in the state of Nebraska or ability to obtain prior to start
• Ability to work across both SNF and home health care settings
• Proficiency or willingness to learn Net Health (SNF) and HCHB (Home Health) documentation platforms
• Must live within 80 miles of the facility to be eligible for local placement consideration

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or home health setting
• Experience supervising PTAs or other therapy support staff
• Familiarity with productivity and points-based documentation systems
