Job Title: Home Health | RN - Registered Nurse
Location: Denver, CO
Setting: Home Health
Pay Range: 2111.57 - 2217.15
Schedule: Discuss with recruiter.
Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements:
Homecare Homebase Software Experience - Required; OASIS Experience - Required; Current state license as a Registered Nurse - Required; CHPN certification - Preferred; Proof of current CPR certification - Required; Current driver's license - Required; Must have reliable transportation and insurance - Required; 1-2 years experience as a Registered Nurse in a clinical care setting - Required; 2+ years Home health experience - Required; Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required; Bachelors of Science in Nursing - Preferred.
Responsibilities:
- Assess home health patients and develop individualized care plans.
- Provide skilled nursing care and education to patients and families.
- Coordinate care with interdisciplinary team members to ensure continuous quality care.
- Monitor and document patient progress, ensuring compliance with third-party requirements.
- Supervise and direct home health aides and other staff providing care.
