Occupational Therapist (OT) Home Health Per Visit / PRN
Penn Home Health Care is seeking a motivated Occupational Therapist (OT) to provide patient-centered care in the home setting.
Service Areas: Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type: Per Visit / PRN
Responsibilities:
- Complete evaluations and reassessments.
- Develop treatment plans focused on activities of daily living and functional independence.
- Provide therapeutic interventions and adaptive equipment recommendations.
- Educate patients and caregivers.
- Maintain accurate and timely documentation.
Qualifications:
- Current Pennsylvania Occupational Therapist license.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Home health experience preferred.
