Bilingual Pediatric Health Navigator (Field-Based)

Health Plan of San Mateo (HPSM)

Health Plan of San Mateo (HPSM) is seeking a Pediatric Community Health Worker to provide in-person care coordination, health education and navigation for infant to toddler members and their families. The role requires travel to member homes, primary care offices and community locations to improve health equity and access to services. The CHW will liaise with providers and social services, document activities in the plan’s systems, and support perinatal and preventive health efforts with a focus #J-18808-Ljbffr Health Plan of San Mateo (HPSM)
