Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Responsibilities Identifies patient discharge needs Assists in implementing patient care strategies to close gaps in medical care upon discharge. Leads daily discharge rounds. Identifies resources for patient self-management planning and discharge. Assists in developing and implementing care plan for medically complex patients. Prepares patient for discharge by ordering home nursing services/IV medications/tube feedings and DME. Other duties as assigned MINIMUM EDUCATION & EXPERIENCE Associate's degree of Applied Science in Nursing (AAS) and 5 years of professional nursing experience required Bachelor's degree in Nursing and Experience in acute care, community health care management, disease management and working with multidisciplinary teams preferred Or equivalent combination of education and experience KNOWLEDGE, SKILLS AND ABILITIES Proficient computer skills, familiarity with electronic medical record required LICENSES AND CERTIFICATIONS RN - Registered Nurse - State Licensure and/or Compact State Licensure in New York State upon hire required The University of Rochester is committed to fostering, cultivating, and preserving an inclusive and welcoming culture to advance the University’s Mission to Learn, Discover, Heal, Create – and Make the World Ever Better.
