Discharge & Care Coordination Case Manager

UF Health

A healthcare organization in Jacksonville, Florida is seeking a Case Manager who will be responsible for developing patient care plans, facilitating discharge planning, and collaborating with a multidisciplinary team. The ideal candidate will hold an Associate degree in Nursing or a Master's in Social Work, have 1-2 years of relevant experience, and possess strong communication and organizational skills. Licensure in Florida as a nurse is preferred for applicants with a nursing background. This role involves working with patients of all age groups. #J-18808-Ljbffr
