RN - Hospice & Palliative Care Nurse
Take on the vital role of developing, implementing, and managing individualized patient care plans within a hospice setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and provide compassionate care. Engage actively in delivering exceptional customer service while fostering respectful relationships with patients, families, and colleagues.
Responsible for creating and managing tailored care plans for hospice patients ranging from newborns to the elderly. Provide skilled nursing services aligned with patient and family goals, focusing on symptom management and comfort. Maintain clear communication with hospice staff and volunteers, ensuring coordinated care delivery.
Develop collaborative care plans based on medical and nursing diagnoses.
Deliver skilled nursing services according to established plans.
Promote symptom relief and comfort in line with patient and family wishes.
Communicate assessments and care updates regularly with hospice team members.
Supervise Home Health Aide services as per policy guidelines.
Complete timely and accurate documentation following agency standards.
Build and sustain positive relationships with patients, families, healthcare providers, and referral sources.
Observe and assess patients' physical, emotional, mental, and spiritual conditions, collaborating with team members as needed.
Adhere strictly to HIPAA regulations regarding patient information privacy and security.
Proficiency with MS Office and keyboarding skills.
Effective communication abilities.
Strong multitasking and prioritization skills.

Graduate of an accredited nursing program.
Several years of clinical nursing experience.
Preferred: Bachelor of Science in Nursing (BSN).
Preferred: Previous hospice and/or home health experience.
Preferred: Certification as a Hospice and Palliative Care Nurse.
Current Registered Nurse license in the state of Kansas.
BLS certification required within three months of hire.
Valid Kansas driver's license and proof of auto liability insurance.
