**Required Experience and Qualifications:** - 2 years of previous experience - At least 1 travel contract completed - Epic experience **Required Licensure and Certifications:** - State license - Driver’s License - BCLS certification **Required Documentation:** - Work history/resume - Skills checklist (completed within 1 year of submission date) - License verification - Copy of required certifications - 1 supervisory reference **Additional Requirements:** - Must not have been an employee of UnityPoint in the last 12 months - Must have a taxable address outside of 50 miles from the facility, unless in Madison where local candidates are accepted with a 10% rate reduction.
