Seven Healthcare is pleased to offer a travel opportunity for an RN – Case Manager in Alexandria, LA. This assignment consists of 8-hour day shifts, 5 shifts per week, totaling 40 hours, with a weekly pay of $2,080.

Key Responsibilities of the RN – Case Manager:
Coordinate and manage patient care plans from admission through discharge
Assess patient needs and develop individualized discharge and transition plans
Collaborate with physicians, nurses, social workers, and interdisciplinary teams to ensure continuity of care
Facilitate utilization review and ensure appropriate resource management
Communicate with patients and families regarding treatment plans, care options, and community resources
Arrange post-acute services including home health, rehabilitation, skilled nursing, and specialty care referrals
Monitor patient progress and address barriers to achieving optimal outcomes
Maintain accurate documentation in compliance with regulatory and facility standards
Requirements:
Minimum 2 years of recent RN Case Management experience
Active Louisiana RN license or Compact RN License
BLS certification required
CCM (Certified Case Manager) certification preferred
Strong knowledge of discharge planning, utilization review, and care coordination processes
Excellent communication, organizational, and problem-solving skills
Ability to work independently and collaboratively in a fast-paced healthcare environment
