Staff - Registered Nurse (RN) - Hospice - $74K-88K per year

Overview Gentiva is seeking a Registered Nurse (RN) Hospice for a nursing role in Jackson, Ohio. This position is ongoing and staff-based. Compensation and Benefits Salary Range: $63,370–74,505 per year. Location Heartland Hospice, Jackson, Ohio. Responsibilities Serve as the primary point of contact for patients and families, guiding them through their hospice journey. Deliver and document skilled, hands‑on nursing care based on individualized Plans of Care in patients’ homes or assisted living settings. Perform initial and ongoing assessments; monitor patient condition and adjust care as needed. Collaborate with interdisciplinary teams (physicians, LPNs, CNAs, social workers, chaplains, etc.) to build and refine personalized care plans. Provide emotional, physical, and spiritual support to patients and their loved ones, including education, comfort, and bereavement guidance. Teach caregivers safe and confident care techniques, offering practical training and emotional reassurance. Communicate important patient condition updates to the care team, ensuring timely interventions. Maintain accurate clinical documentation and follow infection control and compliance standards. Support quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care. Help patients and families navigate medications, hospice philosophy and services, symptom and pain management, and end‑of‑life processes. Conduct comprehensive patient assessments and provide education to patients, families, and caregivers as appropriate. Coordinate including consultation with physicians and nurse schedulers. Document timely clinical records on all patients. Recognize and support home health, palliative and hospice referrals and transitions in levels of care. Participate in on‑call and patient support activities as required. Minimum of one year of RN experience in acute care, oncology, long‑term care, hospice or home health. Hospice or palliative care experience preferred. Current CPR certification. Valid driver’s license and automobile insurance. Knowledge of terminally ill patients and hospice roles; excellent patient assessment and documentation skills.
