Breaking Ground's Street to Home outreach program seeks a part-time Social Worker in Brooklyn to provide clinical assessment and support for outreach teams serving the homeless. Reporting to Team Leader II or Supervisor, you will conduct 9.58 assessments, apply Harm Reduction and Housing First approaches, and coordinate with Housing Advocates to connect individuals with housing and services. #J-18808-Ljbffr Breaking Ground
