Registered Nurse RN Home Health (PRN Weekends)

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Weekend Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in west Pasco County (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas).

Locations: Home health visits in west Pasco County (including New Port Richey, Hudson, Holiday, Trinity, Odessa , and surrounding areas)

Schedule : PRN Weekend role. Must be available to work weekends (Saturdays and/or Sundays). Potential for role to grow into a full-time Weekend Baylor role in the future!

What you'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
