Job Summary:
Physical Therapist Outpatient (PT OP) position in an outpatient setting located in a community healthcare environment. This role involves evaluating patients, developing and implementing personalized treatment plans, and supporting patients return to daily activities. The outpatient therapist will also serve as an educator to patients, families, and peers, ensuring compliance with organizational and professional standards.

Job Details:

• Schedule: Days shift, with a work schedule of 8 hours per day and a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification.
• Preferred Qualifications: Not specified.
• Float: Float rotation may be required.
• Additional Information: Evaluates assigned patients and develops, modifies, and implements treatment plans aligned with physician directions and patient needs. Completes documentation accurately and timely, including goal writing, goal assessment, and charge entry, in compliance with state, federal, and organizational policies. Participates in student training and professional development activities. Ensures all care is provided under valid physician orders and within insurance parameters. Maintains licensure and adheres to state practice acts and professional codes of conduct. Demonstrates organizational performance improvement awareness and actively participates in quality initiatives.
