We understand the realities of home health documentation, scheduling, and compliance, and we've built strong systems and a collaborative team so you can focus on what you do best: caring for patients.If you're an experienced RN looking for a rewarding weekend position with autonomy, flexibility, and a supportive clinical team, we'd love to meet you.About the RoleAs a Baylor Weekend Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes throughout the West Palm Beach to Delray Beach service area. You'll work independently in the field while receiving ongoing support from our Clinical Managers, QA team, and office staff.Key ResponsibilitiesPerform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.Complete comprehensive assessments and develop individualized plans of care.Provide skilled nursing services, including medication management, wound care, patient education, and chronic disease management.Accurately complete OASIS and clinical documentation in our EMR in a timely manner.Collaborate with physicians, therapists, and interdisciplinary team members.Educate patients and caregivers to promote safety, independence, and improved outcomes.Adhere to all Medicare, ACHC, and agency policies and procedures.Active RN license in the state of Florida (required)1 year of Medicare home health experience strongly preferredExperience with Wellsky/Kinnser preferredOASIS experience preferredCurrent CPR certificationValid driver’s license, reliable transportation, and active auto insuranceStrong clinical assessment and time-management skillsComfortable working independently while staying connected to a teamWhat We OfferCompetitive pay per visit or salary optionsFlexible scheduling and manageable territoriesSupportive leadership and accessible clinical managementDedicated QA support to help with documentation and complianceStreamlined EMR and strong operational supportOpportunities for growth and long-term stabilityA respectful, team-focused culture that values your time and expertiseWhy Senior Care Solutions?We believe great patient care starts with taking care of our clinicians.
