Position Overview

We are seeking a licensed Travel Physical Therapist to join our Home Health team, providing high-quality therapy services to patients in their homes across multiple counties. This role offers a weekly hour guarantee, mileage reimbursement, and a consistent Monday-Friday schedule.

Key Details

Weekly Hour Guarantee: 32-40 hours
Productivity Expectation: 30 points
Schedule: Monday-Friday, 8:00 AM-5:00 PM
On-Call / Weekend Requirements: None
Coverage Area: Cambria, Northern Cambria, Indiana County
Mileage Reimbursement: Yes - reimbursed between visits, from first patient to last
Work Setting: Patient homes; travel required between multiple locations daily
Rate: up to $52.00 per hour

Primary Responsibilities

Deliver physical therapy services to patients in their homes in accordance with the plan of care.
Conduct comprehensive patient assessments and develop individualized treatment plans.
Provide therapeutic interventions to improve mobility, strength, and functional independence.
Document all services accurately and promptly in compliance with agency and regulatory standards.
Communicate effectively with patients, caregivers, and interdisciplinary team members.
Travel between patient homes throughout the assigned counties.

Required Qualifications

Active PA Physical Therapist License
Home Health Care experience
Valid driver's license and reliable transportation

Preferred Skills

Experience with HCHB (Homecare Homebase) EMR
Strong clinical judgment and ability to work independently in the field
Excellent communication and time-management skills
