Position Overview We are seeking a licensed Travel Physical Therapist to join our Home Health team, providing high-quality therapy services to patients in their homes across multiple counties. This role offers a weekly hour guarantee, mileage reimbursement, and a consistent Monday-Friday schedule. Key Details
- Weekly Hour Guarantee: 32-40 hours
- Productivity Expectation: 30 points
- Schedule: Monday-Friday, 8:00 AM-5:00 PM
- On-Call / Weekend Requirements: None
- Coverage Area: Cambria, Northern Cambria, Indiana County
- Mileage Reimbursement: Yes - reimbursed between visits, from first patient to last
- Work Setting: Patient homes; travel required between multiple locations daily
- Rate: up to $52.00 per hour
- Deliver physical therapy services to patients in their homes in accordance with the plan of care.
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide therapeutic interventions to improve mobility, strength, and functional independence.
- Document all services accurately and promptly in compliance with agency and regulatory standards.
- Communicate effectively with patients, caregivers, and interdisciplinary team members.
- Travel between patient homes throughout the assigned counties.
- Active PA Physical Therapist License
- Home Health Care experience
- Valid driver's license and reliable transportation
- Experience with HCHB (Homecare Homebase) EMR
- Strong clinical judgment and ability to work independently in the field
- Excellent communication and time-management skills
