ProHealth Group | Home Health & Hospice Care Transition Coordinator Job Description Reports to Director of Marketing and/or Region Manager. Job Summary Care Transition Coordinators serve as the primary liaison between the company and referral sources. They develop, maintain, and service community relations across several settings, providing education and information about the company’s programs and services to patients, caregivers, physicians, case managers, discharge planners, social workers, and others. CTCs are responsible for obtaining patient referrals within the assigned territory for the company. Job Qualifications Education Required: An Associates Degree in Business, Nursing, Marketing, or Healthcare related field. In lieu of a degree, five or more years of experience in business-to-business and/or industry selling experience. Desired: Bachelor’s Degree in Business, Nursing, Marketing, or Healthcare related field. Formal sales training also preferred. Experience Required: One year of sales experience. One or more years of related nursing experience may be substituted for sales experience. Desired: Two to five years of sales experience. One or more years of sales or nursing experience in home health or hospice. Licenses, Certifications, and Registrations Current Driver’s License Current Automobile Liability Insurance Essential Assets Reliable Transportation Cell Phone Specialized Knowledge and Skills Medicare guidelines for home care services Communication and relationship development skills Problem identification, resolution, and strategic planning Communications, public relations and health education Basic knowledge of sales Community assessment identification related to geriatric needs Knowledge of physician, hospital, skilled nursing facility, case management, and discharge planning service needs Aptitudes and Personal Traits Effective communication of homecare and hospice services with all levels of the healthcare industry Accept responsibility for direction, control, and planning of activities Handle stressful situations and deadlines Strong communication skills Promotes team efforts and works well with others Recognizes rights and responsibilities of company and patient confidentiality Manage multiple assignments and redirect priorities on short notice Travel within assigned territory and to sales meetings Primary Job Functions Manage Sales Performance Monthly home health and/or hospice budgets are met consistently. Review territory regularly and discuss optimization with manager. Prepare and participate in sales meetings to achieve company goals. Territory Management Target assigned territory efficiently, communicating and educating about services while using available resources. Develop and implement weekly and monthly call plans for optimal productivity. Strategic planning for territory and identifying new referral sources. Cover territory on weekly, monthly, and quarterly basis. Maintain knowledge of all systems tracking referrals and sources to discuss trends. Meet daily and weekly call quotas. Maintain accurate assignment reports. Customer Knowledge and Market Dynamics Know referral source businesses and practices, organizational structure, and key decision makers. Understand market conditions and trends, including opportunities and challenges. Assess market potential and differentiate between physician and facility opportunities. Understand strengths and weaknesses of home health and hospice competition. Determine referral source potential and maintain productivity. Strong Selling Skills Pre‑call planning and objectives are appropriate and demonstrated. Knowledge of sales process. Relationship building and maintenance. Use sales material, branch staff, medical directors, and marketing budget appropriately. Problem solve and resolve conflict. Administrative Duties Prompt attendance in all meetings. Complete assignments on time. Document sales calls promptly and submit by Monday 8 a.m. Log expenses and report by deadline. Follow up on all pending referrals. Complete timesheets and mileage by Monday 9 a.m. Working Conditions Travel by car to physician offices, hospitals, community centers, and events, with exposure to all extremes of weather. Environments may be unpredictable and not under the control of the Agency. Physical Requirements Perform limited to full-range heavy work requiring sitting, standing, walking, and driving. Possess vision, hearing, and physical dexterity necessary to maintain a driver’s license and operate an automobile. Climb stairs, access and operate elevators, and maneuver on all types of terrain. Communicate effectively orally and in writing, giving and receiving instructions in person and via telephone.
