Certified Home Health Aide (CHHA) - Hourly

Home Health AideThe home health aide provides personal care and related services in the home. She/he functions under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor. The CHHA reports directly to the General Manager. Occupational Exposure Category: A: High exposure to blood borne pathogens.Qualifications:Completion of the 9th grade; prefer high school graduateAbility to read and write in English, consistent with job requirementsCompletion of a PA state certified CHHA course training and competency and been skills competency tested and has an active CHHA certification.Completed a criminal background check and other checks required by law.A reliable means of transportation to get to assigned client visits.Prefer 1 year experience as a CHHA in a health-related setting.Responsibilities:Tasks to be performed by an CHHA must be assigned by and performed under the supervision of an RN who will be responsible for the client care provided by the CHHA.To assist with ADLS and ADLs as per the POCTo follow the POC for each client and document the sameHelps the client to maintain good personal hygieneAssists in maintaining a healthful, safe environmentPlans and prepares nutritious meals.Assists the client with ambulation as per the POCAssists with assigned treatments as ordered by the physician and approved and supervised by the nurseAssists the therapy personnel as needed with rehabilitative processesMay cue for medications ordinarily self-administered as assignedTaking and charting vital signs as per the care planEncourages the client to become as independent as possible according to the nursing care planAttempts to promote client's mental alertness through involvement in activitiesGives simple emotional and psychological support to the client and other members of the householdEstablishes a relationship with client and family which transmits trust and confidentialityPrepares a report of her visit on the day it is performed and incorporates same in the clinical record weekly or as directedReports any change in the client's status or home situation to her immediate supervisor, the staff nurse, or the aide supervisorCarries out their assignment as instructed by the nurse or the paramedical team and documents and reports to the nurse when she is unable to do soWorks with personnel of other community agencies involved in the client's care as directed by the staff nursePerforms routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurseAttends in-service as required by state and federal regulationsConfirming on a weekly basis, the scheduling of visits with the Supervisor/Director, to coordinate necessary visits with other personnel.Notifying Haddon Home Care of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with Haddon Home Care.Wears Haddon Home Care's name badge at all time they are seeing Haddon Home Care clients.Functional Abilities:Must be able to read 12 point or larger type.Must be able to hear and effectively communicate in English.Must be able to stoop and bend.Must be able to travel to prospective clients' residences.Must be able to lift, turn, and transfer clients weighing up to 200 poundsMust be able to carry bundles weighing up to 10 pounds upstairs.Special Skills/Certifications Required: CHHASpecial Equipment To Be Operated: Computer, office equipment (phone, fax, copier, etc)Special Employer Policies or Limitations To Be Required: None
