Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.QualificationsEducation and experience, essentialMust be a graduate of an approved school of clinical educationMust be licensed in the state where they currently practice.Must have at least one year experience in home care.Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.Must have basic demonstrated technology skills, including operation of a mobile device.Education and experience, preferredDemonstrated experience with and knowledge of hospice is preferred.RequirementsMust possess a valid state driver licenseMust maintain automobile liability insurance as required by lawMust maintain dependable transportation in good working conditionMust be able to safely drive an automobile in all types of weather conditionsAdditional InformationEnhabit Home Health & Hospice is an equal opportunity employer.
